Tell your doctor’s office that you want a copy of your Medical chart. Your condition will be documented in your chart and you make copies for yourself and attach a copy to your Affidavit. That way you don’t need your doctor to sign anything, he already had to sign each dated documentation in your chart.

They HAVE TO provide you with a copy of your entire chart. IF you were ever hospitalized after you were diagnosed you can also ask the hospital to mail you a copy of your Medical records. If they ask you why you want to just tell them that you want your home file to be complete.
